This post will support the work of the community discharge Hub covering the Fife community hospitals and Fife patients in Ninewells hospital, Dundee (when required). It involves comprehensive assessment and complex planning for discharge for patients who are in Fife community hospitals. It involves daily involvement with wards and all partner agencies. You will need to bring a wide range of experience from across Health and/or social care with a background in Nursing or AHP.
This post requires the post holder to have a flexible approach as you will work alongside the existing community PFC team, providing additional support where and when required. You will also provide cover for planned and unplanned leave. Based in one of the community hospitals in N E Fife, you will be required to travel between sites providing the service where demand is greatest.
This is a fast paced and exciting post working in a diverse and often challenging environment. You will need to have both an organised and flexible approach to work in this post. A good team player who is confident in their role. You will have a relevant qualification and experience in a relevant area. You will possess excellent communication and organisational skills; demonstrate evidence of effective multi-agency working; and be able to work on your own initiative, plan and manage own workload but also function well within our team. Advanced knowledge and skills in the use of Microsoft Office and associated databases (e.g Excel) are also required for this post. All secondments to be agreed with existing line manager before applying.
NHS Scotland is reducing their full-time working week from 37.5 to 37 hours per week from 1 April 2024 but with no change in pay. This reduction will also be applied pro rata for part time staff. This advert and any subsequent offer/contract of employment therefore reflects the new working hours. However, as not all service areas will be able to adopt the 37 hour working week immediately from 1 April 2024, you may be required to work up to an additional 30 minutes per week for a temporary period for which you would be paid until the service you are working in changes rosters or working patterns to accommodate the new reduced working week.
